Quality Reporting on Registered Products

The National Pharmaceutical Regulatory Agency (NPRA) is committed to ensuring the safety, quality, and efficacy of all pharmaceutical products in Malaysia. Your vigilance and reports on product quality are crucial to our mission.

1. Verify Product Registration Status

Before making a report, it's important to determine if the product is registered with the NPRA.

  • How to Verify:

    • Visit the QUEST3+ Product Search.

    • You can search by product name, registration number (e.g., MALXXXXXXX), or other details.

    • A product is considered registered if it appears in the QUEST3+ database with an active status.

2. Reporting Quality Issues for Registered Products

If you encounter a quality issue with a product confirmed as registered with NPRA, please follow these steps:

  • What to Prepare:

    • Product Details: Full product name, brand name, registration number (MAL No.), batch number, manufacturing and expiry dates, name of manufacturer/distributor.

    • Issue Description: A clear, detailed description of the quality defect (e.g., discoloration, foreign particles, faulty packaging, unusual smell/taste, unexpected appearance).

    • Supporting Evidence (if possible): Clear photographs or videos of the product, its packaging, the defect, and the expiry/batch details. Keep the product and its packaging for potential inspection.

    • Your Contact Information: Name, phone number, and email address.

  • How to Make a Report :

    • Where to find the form: Download the Borang Aduan Kualiti Produk Berdaftar (N3-FR-68).

    • How to send the form:

      • By Email (softcopy): Complete the form electronically, save it, and send it via email to qpr@npra.gov.my.

      • By Post or Walk-in: Print and complete the form, then submit it to:

3. Reporting Quality Issues for Unregistered Products

If you encounter a product that you suspect is unregistered (i.e., it does not appear in the QUEST3+ Product Search or appears to be fraudulent/illegal), the reporting channel is different.

  • How to Make a Report for Unregistered Products:
